1. Services
Performance Collective offers physical therapy, virtual rehab coaching, wellness memberships, performance training, and related health and wellness services. Services are provided by licensed physical therapists and qualified health professionals in accordance with applicable law.
Use of our website does not create a provider-patient relationship. A clinical relationship is established only upon completion of intake documentation and initiation of care.
2. Eligibility
You must be at least 18 years of age to use our website and services. Minors may receive services only with the consent and involvement of a parent or legal guardian.
3. Appointments and Cancellations
Appointment scheduling is subject to provider availability. We ask that you provide at least 24 hours notice to cancel or reschedule an appointment. Late cancellations or no-shows may be subject to a cancellation fee as outlined in your service agreement.
4. Payment and Fees
Fees for services are outlined in the applicable Fee Schedule or service agreement provided at intake. Payment is due at the time of service unless prior arrangements have been made. We reserve the right to update fees with reasonable notice.
Performance Collective is not a participating provider with insurance plans. Patients are responsible for all service fees. We may provide documentation to assist with insurance reimbursement claims, but we do not guarantee reimbursement.
5. SMS and Electronic Communications Consent
By providing your phone number and checking the consent box on our intake or contact forms, you expressly consent to receive text messages (SMS) from Performance Collective, including appointment reminders, care follow-up messages, and informational communications.

• Standard message and data rates from your mobile carrier may apply.
• You may opt out at any time by replying STOP to any message.

Consent to receive SMS messages is not required as a condition of receiving services.
6. Not a Medical Emergency Service
